Plot

While pursuing his quest for the Dark Tower, Roland, the Last Gunslinger is severely injured from attacks by strange lobster-like creatures. He fights both sickness and the first man he brings back to his world, Eddie Dean: a drug addict who is in the process of smuggling cocaine for the drug lord Enrico Balazar. Roland saves Eddie from police capture when he was caught smuggling drugs in an airplane in the year 1987. Suffering from drug withdrawal—drugs that don't exist in Roland's world—Eddie resents Roland and the new life thrust upon him, but the two continue the quest and find a second person—a schizophrenic woman who causes more than a few problems for them all. Odetta Holmes is a black woman with both a good side and an evil one, the latter known as Detta Walker. She lost both her legs in August 1959, and ever since then, the evil Detta has been coming forward more frequently. The two sides battle themselves, along with Roland and Eddie, with interesting consequences. Eventually, through Roland's heroic actions, the two personalities combine to form a new person: Susanna Dean. The third person they encounter turns out to be an adversary, not a companion: Jack Mort, a sociopath who takes sadistic pleasure in injuring and killing random strangers—and the man responsible for the head trauma that created Odetta Holmes's alternate personality, the loss of Odetta/Detta's legs, and the death of Jake Chambers. Roland thinks this man should be punished, rather than being brought into his world. Eddie and Susannah are all part of Roland's ka-tet—defined as "one made from many" and "sharing the same destiny"—and, thus, are essential for Roland to continue his quest; however, Roland is acutely aware that he may need to sacrifice them in order to reach the Dark Tower.
